Financial Referrals
Carewell is not able to directly provide funds for persons in need. However, we can assist persons with limited financial means via referrals to the Philippine Cancer Society, the Andres Soriano Foundation, the JPIC Hospital Ministry, the Philippine Charity Sweepstakes Office (PCSO), the Philippine Amusement and Gaming Corporation (PAGCOR), and other individuals, groups, and organizations who serve the Philippine cancer community.
In order for Carewell to best assist you, please provide us with the following documents:
- Letter FROM PATIENT or FAMILY MEMBER requesting financial assistance
- Medical Certificate from your hospital
- Clinical Abstract from your doctor (oncologist). This should include: Profile of patient, diagnosis, history, plan of treatment, doctor's report with his/her name, signature and license number
- Certification from Social Service worker stating that you are qualified to receive financial aid for medical treatment (this is usually a CASE STUDY written by the social worker)
- Names of medication needed and prices
Once you have submitted all these requirements to Carewell, we will prepare an endorsement letter for you and, when possible, will facilitate the financial assistance request process.
